OBH Community Partners

OBH works collaboratively with community partners and stakeholders who share a commitment to improving the health of the community they serve. From health fair facilitation to initiatives that focus on chronic disease education and early detection, these collaborations are intended to provide greater access to health care and keep our communities well. COMMUNITY PARTNER CLINICS:

The following seven Federally Qualified Health Centers (FQHCs) include OBH's partners that will expand primary care capacity of community-based organizations by developing a 32-site ambulatory care network with existing community-based providers to increase access to primary and preventative health care services in the highest need areas of Central Brooklyn. These new and renovated facilities are expected to add approximately 500,000 new ambulatory care visits a year, which will more than double the number of visits that currently exist in the area. The FQHCs are Subgrantees of OBH via the Health Care Facility Transformation Program: Kings County Grant.
