Diabetic care for Brooklyn families

Managing endocrinology or metabolism disorders is possible with the help of specialized physicians and staff at our Division of Endocrinology & Metabolism. We treat patients with type 1 and 2 diabetes, thyroid, calcium, adrenal, pituitary, osteoporosis, and sexual/reproductive disorders.

Our role is to help you manage or improve your conditions with education and effective diabetes treatment. In addition, highly skilled doctors and staff will work to create an individualized diabetes self management program to improve your health and normalize your daily routines. Diabetes Care is available at many of our facilities, including Brookdale Hospital Medical Center, Interfaith Medical Center, and our outpatient family care centers throughout the community.

One Brooklyn Health provides Diabetes Care and Treatment at Brookdale Hospital Medical Center's robust Medication Management Clinic, run by experienced PharmDs and primary care providers who are NCQA diabetes providers of excellence. Services include a Diabetes Self-Management Program and a Diabetes Prevention Program. In addition, interfaith Medical Center's Bishop Walker Health Center also offers Diabetes Self-Management Group Programs, a Medication Management Clinic and treatment services.

One Brooklyn Health's Diabetes Center of Excellence at the Pierre Toussaint Family Health Center cares for diabetic patients, helping them achieve their highest level of health. Patients receive diabetes treatment from an interdisciplinary team that consists of:

  • Board-certified endocrinologist 
  • Diabetes Care and Education Specialist (DCES) registered nurse 
  • Nurse practitioner dedicated to the care of diabetes and metabolic disease 
  • Doctor of Pharmacy for medication management 
  • Health coaches 
  • Behavioral health social workers 
  • Registered dietician devoted to the care of people living with diabetes.

Diabetic care services: 

  • Diabetes Prevention Program (DPP) 
  • Diabetes Self-Management Program and education classes (DSMP) 
  • Exercise strategies and nutrition counseling 
  • Insulin pump training 
  • Medical treatment to help you reach your health goals 
  • Remote retina scanning to detect eye disease as early as possible 
  • Training on continuous glucose monitors 
  • Video visits (check with the office to see if this is right for you).
