To access your medical records at One Brooklyn Health, you can contact Health Information Management at 718-240-5034 or email healthinfo@bhmcny.org.

You can also access your healthcare information ANYWHERE, ANYTIME at myonebrooklynhealth.org. Our online tool that provides you with access to portions of your health record, all from the comfort of your home or mobile device. View your medical history, medications, allergies, immunizations, test results, and communicate and send secure messages directly with your healthcare providers. You can also request medication refills, make appointments via the appointment management-Make Appointment, or request proxy access to view child’s information and other family members’ records.

Although there may be fees associated with your requests, it is free of charge for Continuing Care (Sending records to your treating physicians/providers). Record copies are mostly provided in hard copies, mailed to the designated address or on CD’s. We do not send records via emails or upload to flash drives (it is strictly prohibited by OBH policies & procedures). Requestors may elect to have the record copies delivered electronically / sent to the patient portal at myonebrooklynhealth.org. Continuing Care requests can either be mailed to the designated providers or securely faxed (preferably) to your Continuing Care providers as requested.
