MyOneBrooklynHealth.org is our secure online tool that provides you with convenient access to portions of your health record anywhere, anytime. Manage your healthcare information from the comfort of your home or mobile device.
With MyOneBrooklynHealth, you can view your medical history, medications, allergies, immunizations, and test results; communicate directly with your healthcare providers through secure messaging; request medication refills; and schedule appointments using the Make Appointment feature. You can also request proxy access to view your child’s health information and, where authorized, access the records of other family members.
Although there may be fees associated with your requests, it is free of charge for Continuing Care (Sending records to your treating physicians/providers). Record copies are mostly provided in hard copies, mailed to the designated address or on CDs. We do not send records via email or upload to flash drives (it is strictly prohibited by OBH policies & procedures). Requestors may elect to have the record copies delivered electronically / sent to the patient portal at myonebrooklynhealth.org. Continuing Care requests can either be mailed to the designated providers or securely faxed (preferably) to your Continuing Care providers as requested.
