Alternative Housing Program

Alternative Housing Program [AHP] is a 44bed Transitional Housing program approved and funded by New York State Office of Mental Health. Alternative Housing Program is a licensed Congregated Care Level 11 supportive apartment treatment program for individuals with serious mental illness. There are scattered apartments located in Bedford Stuyvesant, Crown Heights and East New York communities in Brooklyn.

Supportive residential program means a program which provides supportive services designed to improve or maintain an individual’s ability to live as independently as possible and eventually access generic housing in the community.

 

To be considered for admission to AHP, individuals must:

  • Have a designated Mental illness diagnosis
  • Must have extended impairment in functioning due to diagnosed mental illness
  • Be substance and alcohol free for at least six months.
  • Social Security Income (SSI) or Social Security Disability Income (SSDI) due to designated mental illness and/ or Public Assistance (PA Level II) benefit.
  • Medicaid eligible
  • Individuals must be able to function in a partially supervised shared living space
  • Reliance on psychiatric treatment rehabilitation and supports.
  • Have a completed and approved HRA2010e application and referral packet that includes Psych evaluation, Psychosocial and current Physical.

 

Alternative Housing Program requires that all candidates’ for admission must be able to:

  • Independently and appropriately follow the medication regimen 
  • Participate in Mental health services 
  • Minimally perform basic needs tasks 
  • Perform basic household tasks 
  • Utilize public transportation 
  • Must be involved in vocational/work/day program activities a minimum of 20 hours weekly.
