One Brooklyn Health has three member hospitals with main campuses: Brookdale Hospital Medical Center (BHMC), Interfaith Medical Center (IMC), and Kingsbrook Jewish Medical Center (KJMC). Brookdale Medical Center
The Department of Pharmacy provides the highest quality pharmaceutical care and pharmaceutical services to its patients. Brookdale’s pharmacy was one of the first hospital pharmacies in the New York metropolitan area to provide services 24 hours/day, seven days per week, decentralized clinical services to patients and to implement unit dose distribution, clinical monitoring of drug therapy, and sterile intravenous solution preparation. In 2012 the department was honored to receive both the ASHP Best Practices award for contributions to medication management in the ambulatory clinic setting as well as the ASHP CEO’s Award for Courageous Service, for services provided in the aftermath of Hurricane Sandy.
Our dynamic department is staffed by pharmacists, residents, technicians, clinical pharmacists and a dedicated management team. Clinical pharmacist specialties include Ambulatory Care, Anticoagulation, Cardiology, Critical Care, Emergency Medicine, Internal Medicine, Infectious Diseases, and Psychiatry. We are also an ACPE-accredited continuing pharmacy education provider. Affiliation with Long Island University’s Arnold and Marie Schwartz College of Pharmacy and Touro College of Pharmacy affords the department on-site faculty members who facilitate an academic setting for students and residents to receive experiential training.
Interfaith Medical Center
Interfaith Medical Center is a 231-bed acute care community hospital that provides pharmaceutical care for the patients of the Bed-Sty and Crown Heights communities. Our newly renovated pharmacy has a state-of-the-art USP 797/800 sterile compounding suite, automated dispensing cabinets and utilizes the Epic system as our electronic medical record.
Our pharmacists dispense approximately 90,000 doses of medication per month to our inpatients and we are proud of our robust pharmacist intervention program that avoids medication errors and prevents adverse drug events before the medications reach the patient. We also maintain a very active role in antimicrobial stewardship as we strive to prevent misuse of antibiotics and achieve the best outcomes for our patients. We have the following inpatient services as well as a very active Emergency Department and Ambulatory Surgery: ICU, Med/Surg, Behavioral Health, Detox, Substance abuse.
Our 340B Drug Pricing Program includes a partnership with 5 community owned pharmacies; providing services such as "meds to beds" as well as assuring that our patients are able to obtain the best medication options with the aid of the discounted program.
Our offsite ambulatory care center, The Bishop Walker Center at 528 Prospect Place which has over 35 specialty clinics-including a specialty for sickle-cell patients and we also have a comprehensive HIV center and Methadone treatment clinic. We have two large dental centers that provide care to our community. We serve as an off-site rotation for the OBH PGY1 Residency Program.
Kingsbrook Jewish Medical Center
The Department of Pharmacy provides 24-hour pharmacy services for the Emergency Department, Acute Rehabilitation unit, Rutland Nursing Home, and various ambulatory clinics. Our dynamic department is staffed by pharmacists, residents, technicians, clinical pharmacists, and a dedicated management team. Clinical pharmacist specialties include Emergency Medicine and Pediatrics. We serve as an off-site rotation for the OBH PGY1 Residency Program.
Brookdale RX and Urgent Care RX are retail pharmacies that serve the BHMC patients, the community patients, and BHMC Assisted living patients. We work to provide the highest quality of care to all our patients. We offer many service to customers. We process and dispense prescriptions promptly, accept most major insurance plans, provide delivery services, provide Blister packing of medications, provide medication therapy monitoring, receive electronic prescriptions, carry OTC as well as some specialty items and accept Flex Spending Cards (FSA).

The mission of the Pharmacy Residency Programs at One Brooklyn Health (Brookdale campus) is to afford residents the opportunity to develop the knowledge, experience, and skills needed to become advanced level health care practitioners. Our program advances the professional practice level and knowledge base of the resident through a progression of experiences tailored to their individual needs. Each level of experience will enable the resident to achieve and strive for the following outcomes:
· The ability to provide a high level of direct patient care based on thorough patient assessment skills, pharmacotherapeutic evaluation, critical thinking and problem-solving skills
· The attainment of a high level of communication skills that include written, oral, and presentation skills
· The development of the skills necessary to foster education and research and to become a lifelong learner
· The ability to assess the healthcare needs of the community in order to promote and facilitate preventive care
· The ability to operate ethically and legally in all practices and to maintain integrity, respect and dignity
Program Purpose
The One Brooklyn Health’s PGY1 pharmacy residency program builds on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training.
Program Description
The PGY1 Pharmacy Residency program is an organized, directed, postgraduate training program in pharmacy practice. The goal of this residency program is to train pharmacists to develop the knowledge and experience necessary to provide pharmaceutical care to patients with a strong emphasis on developing and mastering clinical skills. The residency program also enables pharmacists to gain the skills necessary to become effective pharmacy leaders.
The residency program will commence on July 1st and the term of appointment will be for one year ending on June 30th of the following calendar year. The residency year is conducted primarily in four- or six-week blocks of core and elective learning experiences. Core learning
experiences include Internal Medicine, Hospital Administration, Medication Safety, Critical Care, Emergency Medicine, and Infectious Diseases (conditional on preceptor availability). Elective learning experiences include but are not limited to Advanced Internal Medicine, Advanced Critical Care, Cardiology, HIV/HCV ambulatory clinic, Psychiatry, transitions of care, and anticoagulation clinic. The resident is simultaneously expected to prioritize and work on their ongoing clinical, administrative, didactic, and research activities. The resident works closely with the residency program director and primary preceptors to develop goals and objectives that will assist the resident in expanding skills for current and future practice. Regular meetings with the residency program director ensure that long-term goals of the residency program are fulfilled. Training schedules include flexibility to allow residents to pursue areas of personal interest. Research and scholarly activities are also important aspects of the program.
In addition to the various core and elective rotations, the program also has required longitudinal ambulatory care, staffing, and administrative components. Residents will work closely with the Pharmacy Director and Residency Program Director to develop practice management skills with emphasis on all aspects of clinical pharmacy services. Teaching experience is gained by serving as a co-preceptor for pharmacy students on rotation and during facilitation of Long Island University pharmacy courses. Additionally, the resident has the opportunity to participate in the Long Island University postgraduate pharmacy teaching certificate program with fellow pharmacy residents.

Teaching and Academia Experience

· Precept pharmacy students on clinical practice rotations
· Residents will have the opportunity to obtain a teaching certificate through the Arnold and Marie Schwartz College of Pharmacy and Health Sciences (LIU)

4. Complete all evaluations in PharmAcademic
5. Provide at least 1 ACPE-accredited lecture and at least 6 Grand Round Presentations
6. Complete at least 1 research project approved by RPD and present at a peer reviewed meeting (ASHP Midyear, NYCRC, NYSCHP, etc)
7. Submit a research manuscript suitable for publication
8. Submit electronic portfolio including an updated CV
9. Complete Teaching Certificate program
10. Prepare a drug class review, monograph, treatment guideline, or protocol (Goal R2.1, Objective R2.1.1)
11. Participate in and prepare a medication-use-evaluation (Goal R2.1, Objective 2.1.2)
12. Submit 5 ADE reports through RL6 reporting system
13. Complete assigned minutes responsibilities

PGY2 residency programs build upon Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency training to develop pharmacist practitioners with knowledge, skills, and abilities as defined in the educational competency areas, goals, and objectives for advanced practice areas. Residents who successfully complete PGY2 residency programs are prepared for advanced patient care or other specialized positions, and board certification in the advanced practice area, if available.
Program Description
The One Brooklyn Health's PGY2 Ambulatory Care Residency is an organized, directed, postgraduate training program in ambulatory care pharmacy practice. The goal of this residency program is to enable residents to develop the knowledge and experience necessary to provide pharmaceutical care to ambulatory patients with a strong emphasis on developing and mastering clinical skills in the outpatient setting.
This program will offer the resident the opportunity for longitudinal patient care experiences within our collaborative practice clinics, the Medication Management (primary care, diabetes, heart failure, hypertension, asthma, and chronic obstructive pulmonary disease), Anticoagulation (point-of-care testing) , and Special Treatment and Research (HIV) Clinics. Additional longitudinal experiences include the Psychiatry Clinic, Transitions of Care, Community Outreach, Ambulatory Practice Management, Teaching and Research. Primary responsibilities of the resident will include conducting direct patient visits, managing clinic operations, maintaining and reporting of clinic interventions, and collaborating with the interdisciplinary ambulatory team.
Teaching experience is gained by completing monthly lectures during Pharmacy Grand Rounds for pharmacists, pharmacy residents, pharmacy students, and pharmacy technicians. Residents will also have the opportunity to serve as a co-preceptor for pharmacy students and mentor PGY1 Pharmacy residents.
Program Structure
Learning experiences and duration of each experience are as follows. The structure is subject to change depending on the individual resident’s development plan.

Staffing Requirements
· Vaccinations once weekly on a weeknight
· Additional vaccination/community outreach events may occur on weeknights and/or weekends which residents are required to participate in for full event. May alternate with co-residents.

· Residents will provide at least 1 ACPE-accredited lecture
· Residents will provide at least 6 Grand Round Presentations
· Precept pharmacy students on clinical practice rotations

· Verification regarding completion of PGY1 residency program will be completed during July but no later than July 31st. Verification process will include one or all of the following: PharmAcademic graduate tracking, PGY1 Certificate of completion, and/or direct communication with the PGY1 RPD. If verification cannot be completed due to failure of documentation in PharmAcademic graduate tracking, unable to procure PGY1 Certificate of completion, or PGY1 RPD states that resident did not graduate, then PGY2 Resident will be subject to termination from the program.

· Completion of the orientation checklist including signed attestation of receipt of residency manual
· Licensed as a Pharmacist in New York State prior to starting program, or no later than the first 120 days of the residency start date. If not licensed within 120 days, resident’s dismissal will be at the discretion of the Residency Program Director. Please refer to Licensure and Dismissal policies
· Successful completion of all learning experiences
· Completion of all evaluations in PharmAcademic
· Achievement of ≥75% of objectives set forth by the program
· Achievement of all objectives and goals in R1: Patient Care
· Provision of at least 1 ACPE-accredited lecture in-house or to a wider audience, e.g. a regional meeting
· Provision of at least 6 Grand Round Presentations
· Complete research project(s) approved by RPD
· Obtain IRB approval
· Conduct research
· Present results at a peer reviewed meeting (ASHP Midyear, NYCRC, NYSCHP, etc)
· Submit research manuscript (approved and signed by research advisor and RPD)
· Submit electronic portfolio that also includes an updated CV as approved by RPD
· PGY2 Specific:
o Update all ambulatory care databases (Medication Management,
o Anticoagulation, HIV, and Pharmacy Concierge) until the last week of June
o Submit final disease state tracker (Appendix) that has been approved by RPD based on resident having direct patient care experiences in at least 8 out of 15 areas
o Design or deliver a program for patients that focus on health improvement, wellness, and disease prevention (Goal R1.2,Objective R1.2.1)
o Prepare or revise a protocol (e.g. work flow, scope of practice, collaborative practice agreement, or clinical practice protocol) (Goal R2.1, Objective R2.1.1)
o Contribute to the development of a new ambulatory service or enhance an existing service (Goal R2.1, Objective 2.1.2)
o Inclusion of PGY1 and PGY2 Certificate of Completion in portfolio
