Pierre Toussaint is a neighborhood-based healthcare facility. We partner with our culturally diverse communities to provide a continuum of outstanding services to individuals and families, and we accomplish this through a caring, trustworthy team of medical professionals. Our services are patient-centered and supported by the Patient Centered Medical Home (PCMH) model of care.

 

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA), and others. The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.

 

Practices that become recognized under Patient-Centered Specialty Practice Recognition have demonstrated commitment to patient-centered care and clinical quality through streamlined referral processes and care coordination with referring clinicians, timely patient and caregiver-focused care management, and continuous clinical quality improvement.

 

As a Patient-Centered Medical Home (PCMH), Pierre Toussaint strives to advance patient wellness by offering programs to help patients reach their wellness goals.

 

Diabetes Center Of Excellence:

The One Brooklyn Health Diabetes Center of Excellence is designed to improve care for people living with diabetes and enable patients to achieve their highest level of health. The center provides patients with access to an interdisciplinary team that consists of a board-certified endocrinologist, a Diabetes Care and Education Specialist (DCES) registered nurse, a nurse practitioner dedicated to the care of diabetes and metabolic disease, a PharmD, health coaches, and a behavioral health social worker. Health coaching is a valuable intervention that assists patients in meeting glycemic goals and AIC targets to best manage and address diabetes treatment goals. This intervention also speaks to the goals of the patient-centered medical home, fostering close relationships of trust and patient involvement.

 

How education plays an important part in the program model: The center's unique program collaborates with Health People, New York’s leading peer-education-based disease self-care and prevention community group. This groundbreaking training program comprehensively provides on-site peer-led diabetes self-management to high-risk patients living with diabetes, to improve the health status of the patients and reduce the complications of diabetes.

 

DSMP Program: Diabetes Self-Management Program

The Diabetes Self-Management Program (DSMP) offers an opportunity for patients to meet with a health coach for guidance. Advancing your health and quality of life is our priority.
